Is This Insurance?
Nope - this is not insurance. It's better. You can have peace of mind knowing that your eye care is covered, just like how Prime takes care of all your shopping needs. So, no need to worry about the hassle of insurance, because this is the ultimate solution. It's like having a VIP pass for your eyes!
Can I Use An HSA/FSA Card?
Yes. Memberships are a valid charge.
Are There Deductibles?
No. Your benefits start immediately and membership runs for 12 months from the date you join. No deductibles, no hassles, just savings.
Are There Yearly Maximums?
No. You receive unlimited savings included in your plan.
Are There Exclusions For Treatment?
No. This is another benefit of not going through insurance companies - pre-authorizations are not necessary.
Is This A Subscription?
Yes! This is an affordable subscription where you'll get all the health benefits of regular exams without breaking the bank. Just like a gym membership for your vision.
What Is The Per-Person Pricing For The Family Plan Memberships?
Silver Family Plan - First member is $199. Each additional member (with a minimum of two members) is $179. Gold Family Plan - First member is $249. Each additional member (with a minimum of two members) is $229. Platinum Family Plan - First member is $449. Each additional member (with a minimum of two members) is $429.
What Are The Disclosures?
Our membership plans are provided only to uninsured patients of our practice and are not insurance, pre-payment for future services, payment for access to discounted services, or a qualified health plan under the Affordable Care Act. Instead, our patients elect to make a monthly or annual subscription payment for preventive care versus paying at the time of each service. The membership plan is a direct agreement between the patient and the legal entity of our practice. Membership plan payments must be current in order for the patient to receive the benefits included with their membership plan. Membership plan benefits may not be combined with any other offers or discounts. Payment for additional treatment provided by our practice to the patient is due at the time of service and our membership plans do not cover procedures completed outside of our practice. The term of our membership plans is 12 months from the patient Effective Date and automatically renews on the Renewal Date. Patients can opt-out of membership plan renewals by clicking “Cancel Auto-Renew” in the Member Portal or by contacting our practice directly. Refunds are at the sole discretion of our practice.
